This begins with the Guardian of Takish in the Plane of Growth.

To complete the quest you must give all the below items to the Guardian at the same time:

Slay the Corrupter of Life in Hate and loot his skull.
Kill the Corrupted Faun in Wakening Lands.
Kill the Corrupted Panther in Wakening Lands.
Kill the Corrupted Unicorn in Wakening Lands.
