This quest can only be done after the Researcher's Badge Quest.
1. Find Vegalys Keldrane in South Qeynos again and Hail him. Say 'I wish to further advance', 'what individual that unleashed this plague?' and 'what is still missing?'
2. Hand in your Researcher's Badge to Vegalys Keldrane and receive his seal.

3. Zone into the Qeynos aquaducts by swimming down in the pond outside the Crows Pub and Casino (First right coming from the gate) in North Qeynos. Swim through the system until you can swim up to breath again. Take the first Left and go past the "Go Back" writing on the wall. The "an investigator" is right there (loc 951, 30).

4. Hail him and say to the investigator "It was too late?". Give the seal to the investigator.
5. Go through the invisible door at 530, 0 or 450, 75 or 515, 0.

6. Guard Helminth then spawns (loc 376, -49). In the tunnel leading to the 4 spectres.

7. Say to Guard Helminth 'vegalys sent me' OR 'investigator is dead'
8. Give Guard Helminth the Seal Vegalys gave you and he will give it back. Say to Guard Helminth 'I am ready to complete the mission'.
9. A Corrupt Guard Helminth (level 50) will spawn and a necromancer (level 40). A little later a Rotting Sentry will spawn (level 40) Kill Corrupt Guard Helminth.

10. When you kill Corrupt Guard Helminth, Azibelle Spavin will spawn (loc 439, -264) near the four specters.

11. Azibelle Spavin is mid 50s ShadowKnight and It is possible to pull her without having the Specters aggro. Make sure you Pacify Azibelle as pulling her might also pull a Necromancer in the Necromancers Guild and thats not a smart idea. It is also advised that you Pacify the two Sepsis Rats that spawn just before the Specters as well to make the fight even easier.
12. Loot Head of Azibelle Spavin.
Another head. This is a stinky quest.
13. Return to Vegalys Keldrane in South Qeynos and hand in his seal and Azibelle's head.
14. You receive the Qeynos Badge of Honor:
